ABSTRACT Online information on maternal and child care is widely available, but its quality varies across sources. This thesis develops MomCare, a Vietnamese question-answering system based on Retrieval-Augmented Generation (RAG) that retrieves information from a document collection with identifiable sources. The MomCare knowledge base contains 6,597 chunks extracted from PDF, DOCX, and XLSX documents. Retrieval combines FAISS and BM25 with query-dependent Adaptive Alpha weights. Multi-turn context is managed through Adaptive Context Management, while Query Rewriting and Intent Detection are handled in a single LLM call. Before generation, the system checks query scope, age relevance, retrieved evidence, and selected high-risk requests. On the RAGAS evaluation, MomCare achieves 0.741 Faithfulness, 0.780 Context Recall, 0.704 Context Precision, and 0.572 Answer Relevancy. On 2,213 valid ViMedAQA samples, Retrieval achieves 83.46% Hit@5 and 0.7410 MRR@5. Generation achieves a BERTScore of 82.23, BLEU of 32.11, METEOR of 56.25, and ROUGE-L of 50.24. Intent Detection correctly classifies 190 of 200 queries. The 14 function-level safety and grounding tests all pass. The initial adversarial test meets the expected behavior in 42 of 50 cases. After error analysis and Guardrail hardening, the additional regression set passes 27 of 27 cases and a retest on the same adversarial set passes all 50 cases. No false blocking is observed on the 50 valid queries. In a 10-turn conversation experiment, ACM reduces the average history size from 1,238.1 to 426.4 characters. MomCare is a prototype for information retrieval support and is not intended to replace medical examination, diagnosis, prescription, or direct consultation with healthcare professionals. Keywords: Retrieval-Augmented Generation (RAG), healthcare chatbot, maternal and child care, Hybrid Search, medical information retrieval.
